  • Information reasonably sufficient to permit the service provider to contact you, such as an address, telephone number, and, if available, an electronic mail address at which you may be contacted.
  • A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Procedure Following Receipt of a DMCA Notice

Upon receipt of a valid DMCA notice, All Pennsylvania Forms will swiftly take the necessary actions to remove or disable access to the infringing material. We will also notify the individual or entity that posted the content of the infringement claim and take steps to prevent repeat infringements by the same user.

Counter-Notice

If you receive a notification of infringement related to content you posted on All Pennsylvania Forms, you may respond with a counter-notice. Your counter-notice must include your full name, address, phone number, and a statement that you consent to the jurisdiction of Federal District Court for the judicial district in which your address is located, or if your address is outside of the United States, for any judicial district in which All Pennsylvania Forms may be found, and that you will accept service of process from the person who provided the original DMCA notice or an agent of such person.
